In luna iunie a anului 2016 am participat la Congresul Asociatiei Romane de Chirurgie Plastica si Congresul Societatii Romane de Chirurgie Estetica (care si-au unit pentru prima data fortele) la care au fost prezentate un număr record de peste 220 de lucrări științifice susținute de speakeri internationali, speakeri romani, medici primari, specialiști si rezidenți din peste 25 de țări. Desfășurat sub motto-ul „One speciality. One event. One of a kind” a avut ca tema principala “The complex therapeutic support of plastic, aesthetic surgery and burned patient”, iar multe dintre lucrarile prezentate au fost bazate pe experiența a medicilor români care a urmat după incendiul de la Colectiv. Am prezentat o lucrare stiintifica despre tehnici combinate de chirurgie plastica conventionala si chirurgie plastica regenerativa in vederea rezolvarii unui caz de malformatie nazala. Mai exact, retractia severa de alar, o situatie care ridica una din cele mai mari provocari in chirurgia nasului.

In acesta prezentare va prezentam o tehnica chirurgicala inovativa in doi pasi pentru a rezolva cazurile de retractie alara severa.
Primul pas ( chirugia regenerativa) consta in transplantul de tesut adipos la nivelul alarului contractat. Prin acest lucru obtinem o preventive a retractiei postoperatorii, o mai buna calitate a tegumentului si o vindecare rapida, prin factorii de regenerare pe care tesutul adipos le contine.
Al doilea pas- corectarea deformitatii prin adaugarea unei frege cartilaginoase ( rotate din crus medial) la marginea inferioara a crus lateral si acoperirea acesteia printr-un lambou cutanat alar rotat astfel asigurand lungirea alalrului retractat.
Metoda : pacienta in varsta de 22 ani diagnosticata cu retractile alara stanga severa, congenitala,
S-a folosit o tehinca combinata de chirurgie palstica conventionala si chirurgie plastic regenerative
Rezultate : Retractia alara a fost corectata in acest caz obtinand o imbunatire din punct de vedere functional si estetic. Nu au fost observate recurente dupa aceasta tehnica.Cicatricea de la nivelul incizie s-a vindecat asigurand un rezultat satisfacator.Nu a fost necesara o revizuire a cicatricii.
Concluzii : Aceasta tehnica chirurgicala combinata reprezinta o metoda eficienta si de incredere in cee ace priveste contractia severa de alar. Avanatajele acestei tehnici include prevenirea retractiei secundare, usor de desenat, foloseste lamborile cutanate locale in vederea unei potriviri ale tegumentului. Cicatricea rezultata in urma interventiei poate fi tinuta cat mai discreta prin pozitionarea corecata a acesteia respectand astfel subunitatile estetice ale nasului.”
